A question about : Voucher For Glasses And Contact Lenses

I wondered if someone could help please. I'm sure I saw a link put on here somewhere by Martin to download a form for claiming back some costs towards glasses and contact lenses if you are on benefits.

Does anyone know where it is please or where I can get the form from please.
